POPULATION HEALTH FRAMEWORK
Whether it’s population health management or population health as an approach to research, there is a growing interest and understanding of the general term among health care and public health practitioners. The growing understanding that health care only makes up roughly 20% of an individual’s overall health outcomes and that where they live, learn, work and play contributes more has dictated that health care, public health, government, business and other disciplines pull together to create appropriate, effective and financially efficient solutions to our communities’ health challenges. |

ENGAGEMENT
The Center has developed an expertise in creating environments where community and academic partners can digest evidence and collectively adapt what works into local solutions.
