Wisconsin Coverdell Stroke Program

About

Wisconsin’s Asthma Program at the Wisconsin Department of Health Services has received CDC funding since 2001. Shortly after, the Wisconsin Asthma Coalition Executive Committee was formed when the Children’s Health Alliance of Wisconsin pulled together a group of individuals and organizations to plan and implement Wisconsin’s first asthma summit. Today, the Wisconsin’s Asthma Program and the Wisconsin Asthma Coalition work together to focus on communities with the greatest needs, delivering evidence-based strategies to address asthma control. These strategies include patient education, consistent and correct use of preventive medication, control of environmental factors that affect asthma and coordinating public health and health care in order to provide seamless asthma control services.

Aims

The purpose is to maximize the reach, impact, efficiency, and sustainability of comprehensive asthma control services. Activities fall into three broad categories:
  • Infrastructure Strategies: leadership, strategic partnerships, strategic communications, surveillance, and evaluation.
  • Service Strategies: Expand access to comprehensive asthma control services through home-based and/or school-based strategies.
  • Health Systems Strategies: Coordinate with health care organizations to improve coverage, delivery, and use of clinical and other services.
 
Top priorities include:
  • Maintain and expand asthma surveillance efforts.
  • Facilitate implementation of the Wisconsin Asthma Plan.
  • Build local capacity to identify, evaluate and implement asthma control services.
  • Identify coverage for comprehensive asthma control services
  • Improve the quality of asthma care by payers and health care organizations.

Expected Outcomes

  • The Wisconsin Asthma Program along with other statewide strategic partners have increased input into the plans, programs, and policies of payers and health care organizations.
  • Comprehensive asthma control services are expanded to other populations and areas in the state.
  • More people with asthma have access to guidelines-based care.
  • More people with asthma adopt appropriate self-management practices.
  • Public health and health care services are increasingly linked & coordinated.
  • More communities are prepared to support people with asthma.

Partners

The Wisconsin Asthma Program has an array of community partners, including but not limited to health care organizations, hospitals, schools, local health departments, and coalitions.
