The passage of the Patient Protection and Affordable Care Act (PPACA), Pub. L., No. 111-148, 124 Stat. 119 on March 23, 2010 established four new federal requirements for tax-exempt hospitals under section 501(r) of the Internal Revenue Code. They included the requirement to “Conduct a community health needs assessment (CHNA) every three years and adopt an implementation strategy to meet needs identified in the assessment.”
In addition to conducting a community health needs assessment, tax-exempt hospitals are required to create a Community Health Improvement Plan to address the needs identified in the assessment, implement it and evaluate its success and impact. Since 2010, the Milwaukee Health Care Partnership’s health system members have committed to share funding and design of Community Health Needs Assessments (CHNAs) in six southeastern Wisconsin counties in collaboration with 18 local public health departments. The CHNAs serve as the foundation from which hospitals and local health departments develop their respective community health improvement strategies. In addition to the six southeastern Wisconsin counties, CUPH supports the Community Health Needs Assessments for 9 other counties in Wisconsin. |

The five health system members of the Milwaukee Health Care Partnership (Partnership) concluded a comprehensive community health needs assessment (CHNA) in 2016, revealing chronic disease, mental health, substance abuse, violence and access to care as the leading health concerns for Milwaukee County residents.
